Retatrutide Beyond Weight Loss — What the Trials Actually Show

Retatrutide holds the record for weight loss in a Phase 3 trial. But if that is all you know about it, you are missing most of the story. The trials also show it clearing fatty liver, cutting knee-arthritis pain, cleaning up blood fats and possibly protecting the kidneys — effects that go beyond what weight loss alone would explain.

Three switches in one molecule

Semaglutide flips one metabolic switch. Tirzepatide flips two. Retatrutide flips three, once a week:

The glucagon switch is the one that changes everything. Doctors avoided it for years because glucagon raises blood sugar. Switched on gently alongside GLP-1 and GIP, that problem is covered — and what is left is a liver that burns its own fat. You can see it in trial blood work: a fat-burning marker (beta-hydroxybutyrate) rises two to three-fold, and the bigger the rise, the more liver fat is lost. Its long half-life (about six days) is why one injection a week is enough.

The liver: the headline result

Fatty liver disease (MASLD) now affects well over a third of adults, has almost no approved treatment, and is one of the fastest-growing reasons for liver transplants. In a 2024 Nature Medicine study of 98 people with at least 10% liver fat, retatrutide cut liver fat by:

Most striking of all: at 48 weeks, 93% of people on the top dose had a normal liver again (under 5% fat). The study did not take biopsies, so it cannot yet prove scarring reverses — markers of scarring did fall, and the 4,500-person SYNERGY-OUTCOMES trial is designed to settle it.

Knee arthritis

Every extra kilogram adds about four kilograms of force on the knee with each step, and body fat also releases chemicals that wear cartilage directly. In TRIUMPH-4's study of 445 people with obesity and knee arthritis, knee pain at 12 mg fell by about 75% — more than researchers expected from weight loss alone, which hints at an anti-inflammatory effect inside the joint.

The heart and blood fats

Data from 338 people over 48 weeks showed the artery-clogging profile cleaning up:

It specifically reduced the small, dense LDL particles that burrow into artery walls. That is a strong early signal; the long-term heart-outcomes trial will show whether it means fewer heart attacks.

The kidneys

A pooled analysis of two trials found kidney filtration improved at the higher doses, and protein leaking into the urine — an early warning sign of kidney damage — dropped at the top dose. A dedicated trial (TRANSCEND-CKD) is now running.

Sleep apnoea and the brain

Tirzepatide roughly halved sleep-apnoea events and is approved for it overseas; retatrutide is being tested for the same and is expected to do at least as well. The brain is the newest frontier: rat studies in 2026 showed better memory and less brain inflammation, but there are no human brain studies yet — this is the least proven part of the story.

The honest part

Retatrutide is still investigational — not approved by SAHPRA, the FDA or any regulator — and the big outcome trials are still running. TRIUMPH-4 also flagged a nerve side effect called dysaesthesia (unusual skin sensations such as tingling or burning).
